DRIVE for Health Equity: Improving Type 2 Diabetes Outcomes for Minoritized Communities

The DRIVE program partnered with Baywell Health in Oakland, CA to reduce HbA1c levels among 255 minority patients. Through four PDSA cycles, food distribution, culturally tailored DSMES, and community partnerships, the program achieved a mean HbA1c reduction of 1.0%.
